Information obtained by Emma Hall at Life Flower will be collected lawfully and in accordance with the General Data Protection Regulations (GDPR). With changes implemented in May 2018 under the General Data Protection Regulations (GDPR) this gives individuals the right to be informed about the collection and use of their personal data, which includes the collection of health information. Consent  By contacting me through the subscribe box on the website, email or telephone, signing a therapy intake form or consent form in person or attending an online or in person event you are giving your consent to Emma Hall Life Flower to hold your personal information and to make contact. The information which you supply on the forms will be held together with any clinical records or purchasing information, as applicable. I will use your information to give you the best possible service and for insurance purposes.

Email marketing.With your permission, I will send you emails about my practice and other updates. You can unsubscribe at any time by emailing me at emma@lifeflower.co.uk or writing to me direct Emma Hall, Life Flower  

Storing your information Your information is stored either via an encrypted online form which only I have access to or if I hold a paper form it is in a locked cabinet.  I do not store personal data on any portable devices such as a memory stick. All online documents and applications that contain personal data are password protected.

Retaining your information  You have the right to ask me to anonymise your records at any time if you wish to terminate our working relationship. I am required by law to retain adult records for seven years following our last date of contact and will destroy them with a shredder after that date. Children's records are retained until their 25th or 26th birthday, depending on the exact age of the child when their treatment ended.

Accessing your information Your records are accessed by myself for therapy purposes. You have the right to request a copy of your records at any time. These will be sent by email, in a Microsoft Word document attachment.

Sharing your information I will never share your information without your permission - it will be kept strictly confidential unless there are legal grounds for me to disclose something which you have told me to a third party. I shall always inform you if I feel that disclosure is necessary and seek your permission and co-operation first. With your agreed and written consent there may be instances where your medical information may be shared by the various healthcare providers involved in your on-going care and treatment. Due to Coronavirus (Covid-19), should you contract the virus we are obliged to inform NHS Track and Trace

Complaints If, for any reason, you are unhappy with how I am handling your data, please raise your concerns with me first, so that we can seek a resolution. If following this you are still not satisfied, then you have the right to complain to the Information Commissioners Office.

I may amend this privacy policy from time to time when required by legislation to do so. You should check back periodically to check for any changes. This privacy policy is effective from the introduction of GDPR on 25th May 2018.
